Account Update Form

This account update form should be used to make changes to existing accounts. Do not use this form if you are starting new service.

This form can be used to:

• Add another account user
• Be removed from the account
• Update billing preferences (paperless or mailed)
• Applicant Name Change (due to divorce, legal name change, marriage, etc)
